Beverly Hospital Child Dev. Ctr. employs James LaFave, an on-site certified and/or licensed pesticide applicator (certification/license #: 11852) who may be called on to manage all or some of the necessary INDOOR pest problems that may arise.
In addition, this Daycare has a contract with
Phil Seymour of Waltham Services, 226 Lowell Street.
By signing the end of this indoor IPM plan, the IPM coordinator, Deb Ago, of this Daycare and the Pest Management Professionals described above acknowledge, and agree to the terms of this INDOOR integrated pest management plan.
A .INTRODUCTION
In compliance with the Act Protecting Children and Families from Harmful Pesticides, Beverly Hospital Child Dev. Ctr. on 5/14/2009 3:38:00 PM has prepared the following indoor IPM plan. By centralizing all of the information about this facility’s pest management practices the plan serves as a guide to direct this facility’s IPM coordinator, Deb Ago , about pest control and pesticide use. This plan describes the pest management practices for indoor areas of the Beverly Hospital Child Dev. Ctr. and clearly states it’s pesticide use policies.
A copy of the plan has been filed with the Massachusetts Department of Agricultural Resources (MDAR), and at least one printed copy must be kept on site and made available to the public upon request.
Objectives
The objectives of the integrated pest management program conducted at the Beverly Hospital Child Dev. Ctr. are listed below.

In light of these objectives, the Beverly Hospital Child Dev. Ctr. has selected the following as it’s IPM policy statement:
B.POLICY STATEMENT
It is the centers policy to prevent unnecessary exposure to chidlren and employees to chemical pesticides and reduce the need to rely on chemical pesticides when managing pests. It is the policy of the cneter to only use chemical pesticides when psets have been identified and their presence verified.
C.IPM COMMITTEE
The tasks set before an IPM committee are to:-
| Develop an IPM plan. The IPM plan is in essence, a document that describes the organization and implementation of IPM on school grounds. |

| Communicate about IPM - Facilitate communication within the school about IPM practices. |

| Assist in development of contract specifications. |

| Provide notification to parents about pesticide use. |

D. COMMUNICATING IPM WITHIN THE FACILITY
The Pest Management Professional communicates with the IPM coordinator of the facility. The IPM coordinator then passes this information onto an administrative assistant who decides how the information will be distributed throughout the facility.
Staff/Students communicate with their supervisors or teachers who then pass information onto the IPM coordinator.
E. EDUCATION AND TRAINING OF FACILITY OCCUPANTS & STAFF
- All staff will receive training regarding the ipm plans in place at the center.
- Training will cover pest reporting procedures and prevention strategies.
- Training will be conduct to review IPM plans at departmental staff meetings.

F. INDOOR MONITORING
The IPM committee will evaluate the plan annually. When pests are present, Beverly Hospital Child Dev. Ctr. has chosen an INDOOR monitoring schedule that consists of semiweekly inspections. When pests are absent the INDOOR monitoring schedule will consist of semiweekly inspections.The following technique will be used to monitor for pests: The facility’s contracted Pest Management Professional would conduct regular pest inspections and would then instruct the IPM coordinator as to the proper course of action.

| NON-CHEMICAL CONTROLS |
whenever practical the cener will use non chemical means to control or limit pests and to minimize the sue of pesticides. We have directed Waltham Services to perform a thorough inspection and to provide a report identifying condition that are contributing to our problems. Sanitary and housekeeping deficiencies will be reported as well. Once this is done, a priority list will be generated to optimize a plan of corrective actions. the only pesticides considered for use shall be rodenticides in tap=mper resistant bait boxes, insecticidal baits, ready to use dust, gels, termiticides in the presence of an active termite infestation and only when non chemical alternatives have been determned to be ineffective, certain lower risk pesticides classifies as exempt materials under 40CFR 152.25 (also known as the 25B list substances)

Pesticides are only applied by a certified and/or licensed applicator.

Pesticides are used only when monitoring has shown that pests are present and when the use of the pesticide is justified or in the case of an emergency situation.
