By signing the end of this indoor IPM plan, the IPM coordinator, Anthony Stancato, of this School and the Pest Management Professionals described above acknowledge, and agree to the terms of this INDOOR integrated pest management plan.
A .INTRODUCTION
In compliance with the Act Protecting Children and Families from Harmful Pesticides, Evergreen Center on 11/5/2009 10:16:00 AM has prepared the following indoor IPM plan. By centralizing all of the information about this facility’s pest management practices the plan serves as a guide to direct this facility’s IPM coordinator, Anthony Stancato , about pest control and pesticide use. This plan describes the pest management practices for indoor areas of the Evergreen Center and clearly states it’s pesticide use policies.
A copy of the plan has been filed with the Massachusetts Department of Agricultural Resources (MDAR), and at least one printed copy must be kept on site and made available to the public upon request.
Objectives
The objectives of the integrated pest management program conducted at the Evergreen Center are listed below.

| Manage pests that may occur on facilities to prevent interference with the learning environment of the students. |

| Provide the safest playing or athletic surfaces possible. |
In light of these objectives, the Evergreen Center has selected the following as it’s IPM policy statement:
B.POLICY STATEMENT
Structural and landscape pests can pose significant problems for people and property. Pesticides can pose risks to people, property, and the environment. It is therefore the policy of this school to incorporate Integrated Pest Management (IPM) procedures for control of structural and landscape pests. The objective of this program is to provide necessary pest control while minimizing pesticide use.
C.IPM COMMITTEE
The tasks set before an IPM committee are to:-
| Develop an IPM plan. The IPM plan is in essence, a document that describes the organization and implementation of IPM on school grounds. |

| Evaluate progress of the IPM program. |

| Communicate about IPM - Facilitate communication within the school about IPM practices. |

| Assist in development of contract specifications. |

| Provide notification to parents about pesticide use. |

D. COMMUNICATING IPM WITHIN THE FACILITY
The Pest Management Professional communicates with the IPM coordinator of the facility. The IPM coordinator then posts this information in a common viewing area dedicated to the subject of integrated pest management.
Staff/Students communicate with their supervisors or teachers who then pass information onto the IPM coordinator.
E. EDUCATION AND TRAINING OF FACILITY OCCUPANTS & STAFF
- All members of the IPM Committee have and will continue to attend IPM training seminars conducted by Anthony Stancato. There are and will continue to be representatives from the teaching staff, custodial staff, housekeeping staff and food service staff who will then have meetings with their respective staff members regarding IPM procedures.
- Training has included pest-reporting procedures which is kept at an on-site log book at the school pest tolerance levels have also been discussed. A handout sheet of pest prevention strategies has been reviewed.
- Yearly training with updates has been taken place since 2001 and will continue. The IPM Committee attends an annual training seminar and then meetings are held with all other staff members.
- Pam Mazzuchelli, Paul Mazzuchelli or Jim Scott conducts the yearly training meetings. They also are available for any questions or concerns that may arise during the year and are in frequent contact with the IPM Committee regarding any changes in IPM. The training has been evaluated by question and answer sessions with the staff and is deemed a success when it is realized that all staff members'' questions have successfully been answered.
- Staff members strictly regulate the area where food is consumed. Proper waste receptacles are available at all outdoor activities.
F. INDOOR MONITORING
The IPM committee will evaluate the plan annually. When pests are present, Evergreen Center has chosen an INDOOR monitoring schedule that consists of daily inspections. When pests are absent the INDOOR monitoring schedule will consist of monthly inspections.The following technique will be used to monitor for pests: Reports from facility personnel (pest reporting forms) would prompt the IPM coordinator to setup monitoring traps, which would be used to identity the pest and determine its abundance.
G. COURSE OF ACTION TAKEN FOR INDOOR PESTS
The following pests have historically and/or currently been a problem at Evergreen Center:
The School's IPM approach to managing the indoor pests includes the following actions:

Carpenter ants have historically been a problem. Once again there was a problem this spring but it was very fleeting and they went away. A bait application was made, but no ants were found in the monitors. After following all the recommendations, we are happy to report that there were no incidences of any insect infestation this year that required attention. Once again, we are happy to report that because IPM procedures have been strictly adhered to, there has not been any reported problems at this facility for an entire year.

They are observed on the outside of the building and they were also in the kitchen area. This year the faility was insect free. Again, we are happy to report that this facility did not have any insect infestation above threshold levels that would require action.

| SCHOOL PEST ACTIVITY |
This year there were only several found in the kitchen. This current year there were no incidences of any insect infestations that required attention.

| MONITORING/INSPECTION |
Monitors are set up in the kitchen area and a few were found in the monitors. Visual seeing the ants running around in the sink were observed. Monitors are still set up this year, but there have been no insects of any proportion found in the traps.

| ELIMINATING ACCESS |
Entryways: (ie doorways, overhead doors, windows, and holes in exterior wall openings around pipes, electrical fixtures, or ducts) - Doors are shut when not in use, there is weather stripping on all doors, any openings in walls have been chalked and sealed, screens are repaired on a timely basis, vegetation and shrubs are kept away from the structure.

| ELIMINATING SOURCES OF FOOD AND WATER |
The food and debris is picked up and cleaned as soon as possible. Food is stored in inaccessible containers. Waste is also stored in inaccessible containers and is removed at the end of each day. Faucets and leaks are fixed, food preparation equipment is promptly cleaned, grease is removed from vents, ovens and stoves and screens are used on sink and drain floors.

| ELIMINATION OF SHELTER AND HARBORAGE |
Shrubs on the outside of the building were cut back and mulch was pulled back away from the structure. Rotten wood on the outside of the structure was replaced.

| PESTICIDE USE ATTESTATION: |
Pesticides are only applied by a certified and/or licensed applicator.

Pesticides are used only when monitoring has shown that pests are present and when the use of the pesticide is justified or in the case of an emergency situation.

Only pesticides allowed under the Children's and Families' Protection Act are used indoors.

H. RECORD KEEPING
In the case of Evergreen Center, INDOOOR monitoring records will be maintained through the following technique: The use of forms which will be filled out by the person monitoring the facility

During the creation of this IPM plan, Anthony Stancato has assigned committee member Anthony Stancato with the responsibility of applying for an emergency waiver.
