SHALOH HOUSE employs ERIK RUFF, an on-site certified and/or licensed pesticide applicator (certification/license #: 32983) who may be called on to manage all or some of the necessary INDOOR pest problems that may arise.
In addition, this School has a contract with
DAVID BROWN of CLANCY BROS. PEST CONTROL, 617-464-1305.
By signing the end of this indoor IPM plan, the IPM coordinator, RABBI DAN RODKIN, of this School and the Pest Management Professionals described above acknowledge, and agree to the terms of this INDOOR integrated pest management plan. A .INTRODUCTION
In compliance with the Act Protecting Children and Families from Harmful Pesticides, SHALOH HOUSE on 2/4/2005 9:48:00 AM has prepared the following indoor IPM plan. By centralizing all of the information about this facility’s pest management practices the plan serves as a guide to direct this facility’s IPM coordinator, RABBI DAN RODKIN , about pest control and pesticide use. This plan describes the pest management practices for indoor areas of the SHALOH HOUSE and clearly states it’s pesticide use policies. A copy of the plan has been filed with the Massachusetts Department of Agricultural Resources (MDAR), and at least one printed copy must be kept on site and made available to the public upon request. Objectives The objectives of the integrated pest management program conducted at the SHALOH HOUSE are listed below. -
| Reduce children’s exposure to pesticides and pesticide residues whenever possible. | -
| Manage pests that may occur on facilities to prevent interference with the learning environment of the students. | -
| Provide the safest playing or athletic surfaces possible. |
In light of these objectives, the SHALOH HOUSE has selected the following as it’s IPM policy statement: B.POLICY STATEMENT
STATEMENT OF PURPOSE This IPM PLAN was prepared for Shaloh House to: a) Achieve compliance with the Act to Protect Children and Families from Harmful Pesticides. b) Adopt and implement new Pest Management Standards instituted by the Pest Control Industry. c) To provide coordinated & mutually beneficial communications, documentation management and reporting tools for the City of Boston’s Health Department, 5 Clancy Brothers and Shaloh House. IPM POLICY It shall be the policy of Shaloh House not to allow the use of any pesticide under any conditions by our personnel or staff. Physical exclusion via maintenance corrections and improvements may be performed to prevent pest access or eliminate harborage areas. Additionally, sanitation corrections and improvements may be utilized to eliminate pest food sources. However, should sanitation and maintenance corrections and improvements prove inadequate we shall consult our PMP (Pest Management Professional), Erik Ruff. The PMP will confirm and conclude via the use of monitoring stations or through other accepted subjective or objective basis the presence of an active infestation and the identification of the pest. The PMP will consult with Rabbi Dan Rodkin , Shaloh House IPM Coordinator and determine whether the pest incursion has exceeded pest action trigger thresholds. If action trigger thresholds have been reached and requires a response, our PMP will apply the necessary pesticide in legally accepted quantities, methods and locations as specified by the product label and in accordance with all applicable state and federal laws regarding pesticide use, provided that this IPM PLAN has been posted on the Massachusetts Department of Agricultural Resource web site for IPM PLAN Listings. Monitoring, exclusion, and mechanical means are the only pest control methods allowed until the IPM PLAN is posted on the state internet web-site. IPM is a common sense approach to Pest Control that utilizes a variety of pest control measures that results in an ecologically sound and environmentally responsible approach to reduce the amounts of pesticides used and released in the environment consequently limiting human exposure to such pesticides. Biological, chemical, and physical/mechanical controls are used in conjunction with monitoring measures that result in an effective and comprehensive pest management strategy. Shaloh House is committed to achieving compliance with the Act to Protect Children and Families from Harmful Pesticides and utilizing IPM strategies to control and manage our pest control issues in an environmentally responsible fashion. The tasks set before an IPM committee are to:-
| Develop an IPM plan. The IPM plan is in essence, a document that describes the organization and implementation of IPM on school grounds. | -

| Communicate about IPM - Facilitate communication within the school about IPM practices. | -
| Assist in development of contract specifications. | -
| Provide notification to parents about pesticide use. |
The INDOOR committee members selected for the SHALOH HOUSE are listed below: 1) RABBI DAN RODKIN (Indoor IPM Coordinator)
D. COMMUNICATING IPM WITHIN THE FACILITY
The primary means of communication will be the Log Book. The Log Book shall be located in the Principals office and contains the "Pest Sighting Log". The Pest Sighting Log is the primary means to The primary means of communication will be the Log Book. The Log Book shall be located in the Principals Office and contains the “PEST SIGHTING LOG”. The Pest Sighting Log (second document in the Log Book), is the primary means to facilitate communication between Shaloh House and Clancy Bros. Pest Control. The Pest Sighting Log is used by the school’s IPM Coordinator and IPM committee members to log all pest sightings and evidence; stating the type of pest, evidence (such as droppings), date sighting occurred, location of sighting, and the person who sighted or recorded the sighting.
E. EDUCATION AND TRAINING OF FACILITY OCCUPANTS & STAFF
- IPM training is available through Clancy Brothers if approved through The Shaloh House. At times to be determined by the IPM Coordinator, the teachers, staff and employees of Shaloh House.
- INTEGRATED PEST MANAGEMENT LOG BOOK CONTENTS As referred previously, the Logbook is located in the Principals Office. The Logbook contains: 1) The PEST SIGHTING LOG. The IPM Coordinator, Rabbi Dan Rodkin, or his designee shall log all pest evidence and sightings as described above in the communication protocol. Clancy Brothers PMP shall investigate and address all entries during the service visits. 2) SITE SANITATION AND MAINTENANCE SURVEY SHEETS. Clancy Brothers technicians shall detail the inspection findings of sanitation or maintenance deficiencies during each service visit such as: a) Access holes in walls, floors, ceilings, or foundation that requires repair/sealing. b) Harborage areas like over filled storage closets, old unused equipment or junk stored in basements or against walls, and overgrowth of weeds, grass or shrubs. c) Pest food sources presented and caused by standing water, overflowing dumpsters, improper food storage, and poor daily cleaning practices. d) A cleanliness evaluation of: utility areas; restrooms and locker areas; stock & storage areas; kitchen, dish or pot washing/sink areas; and floors, walls, & ceilings. The IPM Coordinator must review and disseminate the data in the survey, notify the appropriate IPM Committee Member (Custodian, Food Service Manager, and Secretary to contact Facilities Management for work order requests) corrective action necessary to remedy all sanitary and maintenance deficiencies. 3) PEST SURVEILLANCE DATA SHEETS which the PMP maintains, provide the date of survey, location of traps, the condition of the traps, identification of the pest and whether further pest management is required. 4) PRODUCT LABELS & MSDS (material safety data sheets) on all pesticides or chemicals to be used at this work site. These must be available for review by employees, students, parents, or regulatory agencies. 5 Clancy Brothers Pest Control has provided Shaloh House with a Reference Manual. This manual contains extensive information on all likely pests (mice, cockroaches) that may invade the school or attack the student population (e.g.: head lice, wasps), how to most effectively counter such infestations and links to many augmenting web sites and a reprint of the Consumer Information Bulletin for Schools, Daycare Centers and Child Care Centers.
- Training programs will be offered quarterly by Clancy Brothers to the IPM committee which includes the coordinator, food service personal, maintenance personal, teachers and staff.
- IPM training is available through Clancy Brothers if approved by Shaloh House. Clancy Brothers recommends IPM Training at not less than three month intervals.
- IPM COORDINATOR RESPONSIBLE FOR EDUCATIONAL MATERIAL. CLANCY BROTHERS WILL ASSIST THE IPM COORDINATOR IF NECESSARY.
F. INDOOR MONITORING
The IPM committee will evaluate the plan quarterly. When pests are present, SHALOH HOUSE has chosen an INDOOR monitoring schedule that consists of semimonthly inspections. When pests are absent the INDOOR monitoring schedule will consist of monthly inspections.The following technique will be used to monitor for pests: PEST MANAGEMENT SERVICE SCHEDULE The Pest Management Service Goal is monthly Inspection, monitoring, and treatment (as indicated). However, if pest problems or the current sanitary and maintenance contributory factors persist or remain un-addressed, then we shall recommend an increase in frequency for Pest Management Service Visits. Factors that determine the service frequency recommendations are sanitation and maintenance compliance, food storage practices, and general pest “Pressure” (population). G. COURSE OF ACTION TAKEN FOR INDOOR PESTS
The following pests have historically and/or currently been a problem at SHALOH HOUSE: The School's IPM approach to managing the indoor pests includes the following actions:

| SCHOOL PEST LOCATION DETAILS | Evidence of Norway rats and house mice through droppings, holes, and scurrying noises in wall voids in basement and kitchen areas.

| SCHOOL PEST ACTIVITY | The threshold level is very high. Numerous droppings found in basement area and behind refrigerator located in kitchen.
|
| MONITORING/INSPECTION | When IPM Plan has been established the Technician will be placing monitoring stations as well as tamper resistant bait stations in areas inaccessible to children in the kitchen and basement areas.

| ELIMINATING ACCESS | Shaloh House shall be responsible for physical corrections, if and when required, for all maintenance and sanitary issues. All doors leading to outside exterior will have tightly fitting door sweeps to prevent rodent and pest entry into building. 2) All utility pipes leading to all other parts of school will be sealed to prevent rodent and pest access to those areas. 3) Cracks, crevices and holes wil be blocked to prevent pest entry into building.

| ELIMINATING SOURCES OF FOOD AND WATER | 1)Restrict and limit locations where food may be consumed. 2) Thoroughly sweep and wash kitchen and classroom floors at the end of each work day to prevent food particle build-up, a rodent attractant and food source. 3) Clean all food prep and eating surfaces after each meal. 4)Place plastic bag liners in all trashcans and rinse out daily. 5)Empty all trashcans into a covered dumpster at the end of each day. 6) Food stuff & Snacks should be labeled and kept in hard plastic covered rodent proof containers. 7)Students must be closely monitoring to prevent spillage of liquids or spreading of crumbs. 8)Class eating surfaces must be thoroughly cleaned immediately after consumption.

| ELIMINATION OF SHELTER AND HARBORAGE | 1) All stock should be off the floor and on pallets or shelves. 2) All shelves and pallets should be at least 6" from walls and 6“ off of floor. 3)All storage closets will be maintained in an organized fashion and any and all clutter removed.

| NON-CHEMICAL CONTROLS | Exclusion methods will be performed such as blocking holes, crack, crevices and other access routes to prevent any future outbreaks from rodents.
|
| PESTICIDE USE ATTESTATION: | Pesticides are only applied by a certified and/or licensed applicator.
|
Pesticides are used only when monitoring has shown that pests are present and when the use of the pesticide is justified or in the case of an emergency situation.
|
Only pesticides allowed under the Children's and Families' Protection Act are used indoors.

H. RECORD KEEPING
